In an effort to expedite the check in process, please feel free to print and complete the History and Release forms. These forms are collected once a year at your annual well visit or if you are a new patient to this practice. Please present the completed forms to the check in person upon your arrival. If you have not completed beforehand, blank forms will be given to you at the time of your visit. They will need to be completed before you see the Doctor.
